1. THE PARTIES

THE PRINCIPAL: [NOMBRE DEL CLIENTE], hereinafter referred to as "The Owner."

THE AGENT : Alejandro Salvador Anchondo Pérez, acting on behalf of PIX4HEALTH.

2. SCOPE OF AUTHORITY

The Owner hereby grants Alejandro Salvador Anchondo Pérez the authority to collect, on the Owner's behalf, wellness supplies, care products, and personal goods acquired by the Owner at the location(s) specified via electronic means or within the PIX4HEALTH platform.

It is expressly understood that Mr. Anchondo Pérez and PIX4HEALTH provide Specialized Logistical Management Services only. They are not the owners, manufacturers, sellers, nor distributors of the goods, but merely the designated entity to facilitate their physical secure transport, handling, and delivery.

3. LEGAL COMPLIANCE & DECLARATIONS

This mandate is executed in strict adherence to federal laws, international trade standards, and border regulations:

• Personal Wellness Supplies: Any care items or health supplies transported under this mandate are strictly for the Owner's personal use, are non-commercial, and do not exceed a 90-day supply, in strict alignment with FDA (Food and Drug Administration) personal importation and wellness compliance policies.

• Artisanal & Manufactured Goods: It is declared that all artisanal products, goods, or devices transported herein are completely safe and represent zero safety, environmental, or health risks.

• Processed Nutritional Goods: All permitted food or nutritional items are fully processed or vacuum-sealed. In strict compliance with CBP (U.S. Customs and Border Protection) regulations, no live animals, uncertified animal by-products, or hazardous raw agricultural products containing unpermitted seeds are included in this shipment.

4. ELECTRONIC SIGNATURE & CONSENT 

​

The digital generation of this mandate, including the verified timestamp and the User’s unique originating IP address, constitutes a tacit authorization and legal electronic signature. This agreement is fully binding and enforceable under the Electronic Signatures in Global and National Commerce Act (ESIGN), the Uniform Electronic Transactions Act (UETA), and relevant international digital commerce laws.
